The offer is only available where the initial consultation is booked and attended before 23:59 on 31/08/2026. For discounted treatment plans, the treatment plan must be agreed by 31/08/2026. Offer does not apply to single arche cases or Fixed Appliance. Patients must be dentally fit to proceed with Angel Aligner Treatment. Whether a patient is deemed dentally fit for treatment will be at the clinician’s absolute discretion. Additional treatment required to make a patient dentally fit and suitable for Angel Aligners will be charged at an additional cost. Appointments are subject to availability [and are with Dr Usman Qureshi ONLY. The Angel Aligner plan discount is non-exchangeable, non-transferable, and is not redeemable for cash or other treatments. All sales are final. In the event that Dr Usman Qureshi is unavailable for any appointment scheduled in relation to this offer, Portman Smile Clinic Horsham will use best efforts to rearrange the appointment, honouring the offer. Where the Angel Aligner Treatment plan is interrupted as a result of an appointment, or multiple appointments, being cancelled as a result of Dr Usman Qureshi being unavailable and cannot be completed due to continued unavailability, Portman Smile Clinic Horsham will refund the patient the amount previously paid for the treatment. For clarity, where two appointments have been attended and paid for by the patient, and the final appointment must be cancelled by Portman Smile Clinic, and cannot be rearranged, only the amount already paid for the final appointment will be refunded to the patient. The Angel Aligner plan discount cannot be used in conjunction with any other offer. Portman Smile Clinic accepts no responsibility for any damage, loss, liabilities, injury or disappointment incurred or suffered as a result of the use of this offer or receipt of the treatment. Portman Smile Clinic Horsham does not guarantee that the treatment will not cause damage of any kind (whether direct or indirect) to the patient’s teeth, and therefore the practice accepts no responsibility in the event that the treatment causes any damage to the patient’s teeth. Nothing in these terms and conditions excludes or limits liability for personal injury resulting from Portman Smile Clinic Horsham negligence or for fraudulent misrepresentation, or for any other liability which cannot be limited by law. The offer is open to people aged 7 and over. Portman Smile Clinic can vary or replace these conditions from time to time, at its sole discretion and reserves the right to withdraw this offer at any time. If any provision or part-provision of these terms and conditions is or becomes invalid, illegal or unenforceable, it shall be deemed deleted, but that shall not affect the validity and enforceability of the rest of the terms and conditions.
